The Utility Billing Department is responsible for establishing and maintaining all residential and commercial utility customer inquiries regarding new construction, connection, disconnection and transfer of services. The department is located on the first floor inside City Hall, 101 E. Main St. Operating hours are from 8 a.m. to 5 p.m., Monday through Friday, residents may also make after hours payments in the drop box at City Hall near the Main St. entrance.

Water/Sewer Service Request Forms:

New Water/Sewer Service Request 
All new Water/Sewer service requests require a payment of $100.00 which includes an application fee of $25 and a refundable deposit of $75 and a lease, or closing papers for the property. 
Water/Sewer Disconnection Service Request
Disconnection requests received by 12 p.m. can be processed the same day. Refund checks (if applicable) will take 4-6 weeks for processing. 
Water/Sewer Account Change Request
Online form to authorize a change of address, name, adding an account holder or transferring service. All information must be completed with a copy of the account holder(s) drivers license.

New/Temporary/Disconnection Water/Sewer Request Form
Printable form for Residential new, temporary and disconnection service requests. Send completed form by mail, by hand or by email.
Water/Sewer Account Change Request
Printable form to authorize a change of address, name, adding an account holder or transferring service. All information must be completed with a copy of the account holder(s) drivers license.
Transfer Service Form
Transferring Water/Sewer service from a current Forney account to a new location allows for up to 7 days of service at both locations; if more time is needed a New Service Request will be required.  $15.00 transfer fee applies.

All forms must be signed and include a copy of your photo ID/Drivers license to be processed.  Any service requests received after 12 p.m. will not be effective until the next business day.
